This questionnaire aims to identify to what extent your site security management system is complete in comparison to the requirements set in the C-TPAT standard.
 
To facilitate the self-assessment process, the following scoring convention can be used.  Respone of “No” indicates that the organization has failed to address a particular requirement of the standard.  As an example, the standard requires the organization should have process to inspect the reliability of the locking mechanism of the doors of the containers/trailers. A score of “No” would apply if a process had not been established and implemented.
 
A score of “Partially” would apply in instances where the organization has partially implemented a requirement of the standard. As an example, the organization has established and implemented processes for all employees have an identification badge; however, partial employee do not have identifiation badge. 
 
A score of “Yes” would apply if the organization can provide objective evidence that it has completely fulfilled a requirement of the standard. As an example, the standard requires facility should have procedures in place to remove identification, facility, and system access for terminated employees.  And the organization maintains documented information evidences show the factory implemented procedure to remove identification, facility, and system access of terminated employee.
